Brothers and Sisters:

As the Business Representative-At-Large for Local 230, my assignments include helping Local 230 members to resolve any Trust Fund concerns including DC #16 pension, medical, 401, National Pension, vacation and holiday, asbestos exposure, benevolent fund, and all other matters dealing with all the Trust Funds of District Council #16. 

If you tried to contact the Trust Fund at 1/800/595-7473 for assistance (now open on Thursday evenings until 6:00 PM) with any matters concerning your medical coverage, how to minimize your out of pocket expense by using AHF doctors and hospitals, explanation of benefits forms received, coverage for future planned medical or dental procedures, eligibility, dependent coverage, or any other related medical questions, and you are unable to get it resolved by the telephone, I will intercede on your behalf. To intercede on your behalf with the Health and Welfare Trust Fund, it will be necessary for you to complete an Authorization Form listing me as an Authorized Person so the Trust Fund will be able to disclose your Personal Medical Information to me due to the privacy regulations for Protected Health Information enacted by congress to protect us from unwarranted background medical searches.

As for all other matters, I also will be able to discuss with you  information concerning your pension status, vesting eligibility, estimated benefits that you will receive once you retire, options for various retirement spousal survivor benefits, death benefits, and other retirement issues, without any additional paperwork.
